Insurance
I am in network with the following insurance companies:

If I do not take your insurance, I will supply you with a Superbill for reimbursement to your insurance company if you have out-of-network benefits. All insurance companies are different, so you may want to check with your insurance company regarding your benefits. You can maintain your privacy by going private pay as insurance companies cannot request to see your file if you do not go through insurance.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment or do not notify us at least 24 hours in advance, you will be required to pay a cancellation fee of $100.
